Gateway Community Treatment Program
The Gateway Community Treatment Program in Chester offers quality clinical rehabilitation services designed to enable people with psychiatric disabilities to function to the best of their ability. Gateway's case managed clients are in the low to moderate range of care and need on-going long-term assistance with a wide range of activities. At Gateway, clients receive case management, psychotherapy, social rehabilitation, and psychopharmacologic services. Another key factor in the recovery process is the utilization of community-based natural supports. The social rehabilitation program follows well-researched rehabilitation models offering a structured and varied psycho-educational and experiential program. Social rehabilitation also includes curricula that are focused on social skill building, symptom recognition and management, and preparation for community presentation and employment.

Approximately 75% of Gateway's clients live in "board and care" facilities where most of their daily needs are met. The Gateway program pursues recovery goals with these clients that focus on facilitating independent living skills helping them to be increasingly responsible for their own lives. The skill sets necessary for these changes are learned and practiced in Gateway's Social Rehabilitation program and reinforced by case management and psychotherapy. Some clients have achieved the goal of moving out of their board and care facilities into their own apartments and also pursue volunteer or competitive work. The Gateway Community Treatment Program is accredited by CARF.

Assertive Community Treatment (ACT) Team
ACT is an intensive, community-based case management program that offers counseling, support and treatment services to individuals residing in their own homes in the Middletown community. A multi-disciplinary team including a psychiatrist, psychiatric nurses, and case managers provide outreach services ranging from teaching basic living skills such as shopping, cooking, hygiene, budgeting, and transportation, to monitoring medications, psychiatric treatment, evaluation, counseling and relapse prevention. The ACT Team is available to offer direct service Monday – Friday 8am-8pm, and by phone 8 pm – 8am.  Saturdays, Sundays and holidays 10 am – 6 pm and by phone 6 pm – 10 am.
